KPTA is pleased to partner with RehabCare to bring you the 2017 KPTA/RehabCare All Academic Team

 

The purpose of the KPTA All-Academic Team is to recognize outstanding physical therapy student members of the APTA/KPTA who are likely to make a significant impact upon the profession, whether on a community, regional, state or national level and to encourage increased participation at the student level.

Details: 

Applicants may nominate themselves or may be nominated by a fellow student who is a member of the APTA/KPTA ormay be nominated by a faculty member who is a member of the APTA/KPTA.

 

o   Up to three students will be recognized from the following PT Programs:

§  Bellarmine University; and

§  University of Kentucky, including at least one student from the Hazard satellite program.

§  Western Kentucky University.

 

o   Up to two students will be recognized from the following PTA Programs:

§  Hazard Community & Technical College / Southeast Kentucky Community & Technical College (one from each campus);

§  Jefferson Community & Technical College;

§  Madisonville Community College;

§  Somerset Community College; and

§  West Kentucky Community & Technical College.

 

All nominees are required to submit the following documentation in one single application packet:

o   A completed application form;

o   A personal statement about the nominee’s selection of physical therapy as a profession, current challenges facing the profession, and the nominee’s professional goals with regards to leadership within the APTA/KPTA, not to exceed one single-spaced page.  Font must be 10-point or greater.  A footer with the nominee’s name and educational institution must be included.  Applications that do not adhere to these guidelines may not be considered.

 

Application packets must be received by the KPTA Student Liaison by 5:00 PM on May 15, 2017.  Late applications will not be accepted.  The KPTA / KPTA Student Liaison is not responsible for late or misdirected mail.  Applications may be submitted electronically, including the words “KPTA All-Academic Team Nomination” in the e-mail title, to sonia.young@wku.edu adhering to the same application deadline.

  

The application packets will be scored by a committee composed of current APTA members.

 

Applicants will be notified in writing of their selection or non-selection to the team.

 

Application materials will not be returned. 

 

Members of the KPTA All-Academic will be recognized at the 2017 KPTA Kentucky Conclave and in the KPTA Newsletter and will receive a certificate or plaque; monetary awards may be provided depending upon available resources.

 

Each participating academic institution will receive an engraved plaque listing the recipients of the award to be displayed prominently at their institution.  The educational institution will be responsible for the engravings of future student award winners.

Members of the team will be asked to electronically submit a photograph (head and shoulders) for the KPTA Newsletter and for the presentation at the conference once the team is finalized. 

 

All members of the team must submit a completed W-9 in order to receive the RehabCare scholarship.  The W-9 will be sent electronically to all members of the team once finalized.  In addition, scholarship recipients are expected to participate in a blogging experience coordinated by RehabCare detailing their experiences as a PT / PTA student.
